A student needs to give 15 types of information on a unique ID card. Name of the student in Bengali and English, birth registration, date of birth, place of birth, gender, nationality, religion, the class being studied, class roll number, marital status, disability (if applicable), blood group, ethnicity or not, mother and father’s name in English, National Identity Card number, mobile, occupation, whether alive or dead should also be mentioned. To mention the student’s relationship with the parent if the parent is not the parent.
শিক্ষার্থীদের ফরম পূরণের পাশাপাশি নিন্মোক্ত ডকুমেন্ট সংযুক্ত করে দিতে হবে-
- ১. শিক্ষার্থীর সদ্য তােলা পাসপাের্ট সাইজের ২ কপি ছবি (সাদা ব্যাকগ্রাউন্ড রঙ্গিন ছবি, যা সম্মুখভাগ থেকে তােলা যেন দুই চোখ একই সাথে দৃশ্যমান হয়);
- ২. শিক্ষার্থীর জন্ম নিবন্ধন সনদ (Birth Registration Certificate) এর অনুলিপি;
- ৩. শিক্ষার্থীর মাতা, পিতা এবং প্রযােজ্যক্ষেত্রে অভিভাবকের জাতীয় পরিচয়পত্রের অনুলিপি;
- ৪. শিক্ষার্থীর মাতা, পিতা এবং প্রযােজ্যক্ষেত্রে অভিভাবকের জন্ম নিবন্ধনের অনুলিপি(যদি থাকে)
ইউনিক আইডি ফরম পূরণের নিয়ম ও নির্দেশনা:
শিক্ষার্থীরা তাদের ইউনিক আইডি সংগ্রহের জন্য অবশ্যই সতর্কতা অবলম্বন করে ফরমটি পূরণ করতে হবে। সেগুলো নিচে আলোচনা করা হল-
১. শিক্ষার্থীদের মৌলিক তথ্য প্রদান: শিক্ষার্থীরা তাদের ইউনিক আইডি ফরমে তাদের মৌলিক তথ্যাদি যেমন- নাম ও জন্ম তারিখ অবশ্যই তাদের অনলাইন জন্ম নিবন্ধন অনুসরণ করে প্রদান করতে হবে।
কারো যদি অনলাইন জন্ম নিবন্ধন না থাকে তাহলে অবশ্যই রেজিষ্টারের কার্যালয় থেকে দ্রুততম সময়ের মধ্যে জন্মনিবন্ধন সনদ সংশোধন করে নিতে হবে। যা অনলাইন জন্ম নিবন্ধন পোর্টালে থাকবে।
২. পিতা ও মাতার তথ্য প্রদান : শিক্ষার্থীদের পিতা ও মাতার তথ্য অবশ্যই পিতা ও মাতার জাতীয়পরিচয়পত্র অনুসরণ করে লিপিবদ্ধ করতে হবে। যদি কারও তথ্য পিতা মাতার আইডি কার্ডের সাথে না মেলে তাহলে তা সংশ্লিষ্ট কার্যালয় থেকে সংশোধন করে নিতে হবে।
৩. শিক্ষা ও অন্যান্য তথ্যাদি : যথাযথ সতর্কতা অনুসরণ করে শিক্ষার্থীরা তাদের পূর্বর্বতী ও বর্তমান শিক্ষাগত তথ্য প্রদান করবেন। শ্রেণি শিক্ষক ও প্রতিষ্ঠান প্রধান তা দেখে সত্যায়ন করবেন।
